Adult Day Health Services

HEALTH... Treatment and care are individualized under the

direction of each participant's personal physician. Each member

of the program is provided with a

comprehensive care plan. This plan

that may include pharmaceutical,

dental, ophthalmology, and podiatry

services, is coordinated by a registered nurse and implemented

by our multi-disciplinary team of professionals.

• Participants receive all the services provided by a nursing
  
  center and yet are able to return to their home each evening.
  
  
• Isolated adults socialize and interact with their peers in a
  
  community setting.
  
  
• Reducing the caregiver's emotional and physical stress. The
  
  caregiver is secure in the knowledge that their loved one is
  
  receiving nursing, personal care, and other necessary
  
  services, while enjoying recreational activities in a safe
  
  environment.
  
  
• Holistic approach and individualized, comprehensive planning
  
  assures that members receive proper care and services.
  
  

ADMISSION... After an application is submitted, applicants are

personally interviewed to determine if they can benefit from the

Adult Day Health Care Services program. There are only two

criteria necessary for program eligibility:

• Medical condition
  
  
• Medical coverage or Direct pay
