Short-Term Rehabilitation / Sub-acute Care Services

At Carillon Nursing and Rehabilitation Center, we share your goal

of returning home as soon as possible. We recognize, however,

that sometimes a hospital stay is not long enough to provide

optimal recovery. Our short-term unit,

offering rehabilitation and sub-acute

care services, was designed to bridge

the gap between the hospital and home. Our facility offers an

independent short-term unit, fully equipped to provide all the

comforts of home. Private and semi-private rooms are available.

As a short-term care patient, you will participate in an individ-

ualized aggressive treatment plan tailored to your own needs and

abilities, always with the ultimate goal of independence. Our

interdisciplinary team approach utilizes the expertise of our

physicians, therapists, nurses, case managers, social workers,

and discharge planners to provide the highest quality care during

your stay. Our staff is professional, kind and caring, trained

to meet complex care needs and dedicated to working with you

every step of the way as you travel along your road to recovery.

As your rehabilitation progresses, regular meetings will be held

for you and your family to discuss your progress. Soon we will

begin to look toward preparing for your return home. And, be

assured, we will secure appropriate support services to assist you

once you get there. At Carillon, helping you get back home is our
